Tóm tắt những kết luận mới của luận án:

Trên cơ sở nghiên cứu 47 trạm y tế và phỏng vấn 235-238 nhân viên y tế và 471-474 người dân về chất lượng khám bệnh, chữa bệnh bảo hiểm y tế tại các trạm y tế xã 2 huyện Mai Châu và Tân Lạc tỉnh Hòa Bình, chúng tôi có một số kết luận sau:

1. Đã khái quát được khái niệm “chất lượng khám bệnh, chữa bệnh bảo hiểm y tế tại trạm y tế xã” và từ đó, đã mô tả được thực trạng chất lượng trên tại 47 trạm y tế xã 2 huyện nghiên cứu đạt khá (53,2% trạm có bác sỹ, 82,6% thôn bản có nhân viên y tế; 88,1% dân số có thẻ bảo hiểm y tế với số lượt khám trung bình 1,18 lượt/thẻ/năm; tỷ lệ nhân viên y tế và người dân 47 xã của 2 huyện hài lòng về chất lượng khám chữa bệnh bảo hiểm y tế ở mức khá, tương ứng là: 74,0% và 74,6%), mặc dù năng lực chuyên môn (53,2% số trạm thực hiện được 61/76 kỹ thuật theo quy định) và điều kiện bảo đảm chất lượng của các trạm y tế xã đạt mức trung bình với việc có xây dựng hướng dẫn (48,9% số trạm) và thực hiện nhiều bước trong Quy trình khám bệnh, chữa bệnh 14 bước đạt ở mức thấp và rất thấp (4,3-12,8% trạm thực hiện). 

2.Đã xác định được 10 yếu tố từ nhân viên y tế liên quan đến chất lượng khám bệnh, chữa bệnh bảo hiểm y tế tại trạm trong phân tích đơn biến (giới tính, trình độ đào tạo, chức danh lãnh đạo, chức danh chuyên môn, có hướng dẫn quy trình, thực hiện đủ 14 bước của quy trình, có ứng dụng công nghệ thông tin, có sự quan tâm của chính quyền xã, có phối hợp đa ngành) và 3 yếu tố trong phân tích đa biến (trình độ đào tạo, có bác sỹ làm việc và có phối hợp đa ngành). Với người dân, đã xác định được 8 yếu tố trong phân tích đơn biến (trạm y tế xã có hướng dẫn quy trình; trạm đạt Chuẩn Quốc gia; trạm có bác sỹ; quản lý tốt hồ sơ, bệnh án; có ứng dụng công nghệ thông tin; cơ sở vật chất đầy đủ; cảnh quan môi trường bảo đảm, có phối hợp với bảo hiểm xã hội) và 4 biến trong phân tích đa biến (trạm có bác sỹ; có ứng dụng công nghệ thông tin; bảo đảm được cơ sở vật chất và có phối hợp với bảo hiểm xã hội).

3. Làm rõ được hiệu quả của các biện pháp can thiệp (Quy trình cải tiến khám bệnh chữa bệnh bảo hiểm y tế 6 bước tại trạm y tế xã và đào tạo liên tục thông qua các lớp tập huấn trực tiếp hoặc trực tuyến bằng website https://healthvietnam.vn) đã nâng cao năng lực và điều kiện của các trạm y tế xã bảo đảm chất lượng khám bệnh, chữa bệnh bảo hiểm y tế: tăng tỷ lệ trạm thực hiện được từ 80% kỹ thuật chuyên môn, bảo đảm thuốc từ 80% trở lên, ứng dụng công nghệ thông tin vào khám bệnh chữa bệnh; kết quả hoạt động của các trạm y tế xã can thiệp đã tốt lên; kết quả phân tích đa biến DD cho thấy sự hài lòng của NVYT và người dân được can thiệp tăng lên gấp nhiều lần so với NVYT và người dân các xã không nhận được sự can thiệp và tỷ lệ tiếp tục sử dụng dịch vụ y tế tại trạm tăng rất cao đạt 96,1%.

Summary of new findings of the thesis:

Based on the research on 47 commune health stations, interviews with 235-238 healthcare workers and 471-474 people about the quality of medical examination and treatment covered by health insurance at commune health stations in Mai Chau and Tan Lac districts, Hoa Binh province, we have drawn on the following findings:

1. The concept of “quality of medical examination and treatment covered by health insurance at commune health stations” had been generalized. Hence, such quality situation at 47 commune health stations in the two studied districts had been described as satisfactory (53.2% of stations have doctors, 82.6% of villages have healthcare workers; 88.1% of the population have health insurance with an average number of visits of 1.18 visits/health insurance card/year; the number of healthcare workers and people in 47 communes of these 2 districts who were satisfied with the quality of medical examination and treatment covered by health insurance reaches satisfactory percentages of 74.0% and 74.6%, respectively), though the professional capacity (53.2% of the stations can perform 61/76 of the prescribed techniques) and the quality conditions of the commune health stations reached an average level, with low and very low level of guidelines development (48.9% of the commune health stations) and multi-step performance in the 14-step medical examination and treatment process (4.3-12.8% of stations performing).

2. Ten factors from medical staff had been identified with reference to the quality of medical examination and treatment covered by health insurance at the stations in univariate analysis (gender, training level, leadership title, professional title, procedures guidance, complete 14 steps of the procedure, application of information technology, concern of commune authorities, multi-sectoral coordination). Three factors in multivariate analysis (training level, working doctor(s) and multi-sectoral coordination) had also been determined. For the people, eight factors of univariate analysis (commune health stations have procedure instructions; stations meet national standards; stations have doctors; documents and medical records are well managed; information technology applications; adequate facilities and infrastructure; guaranteed environmental landscape, coordination with social insurance) and 4 variants of multivariate analysis (stations have doctors, information technology applications; adequate facilities and infrastructure, and coordination with social insurance) had been identified.

3. It had been clarified that the effectiveness of interventions (6-step procedure to improve medical examination and treatment covered by health insurance at commune health stations and continuous training through offline or online training courses through the website https://healthvietnam.vn) had improved the capacity and conditions of commune health stations to ensure the quality of medical examination and treatment covered by health insurance: increasing the percentage of stations that can perform from 80% specialized techniques, ensuring adequate quantity of drugs at 80% or more, applying information technology to medical examination and treatment; the performance of the commune health stations with intervention had improved; The results of DD multivariate analysis showed that the satisfaction of healthcare workers and people receiving intervention increased many times compared to those in the communes that did not receive the intervention, and the rate of continuing to use health services at the stations had increased greatly to 96.1%.
