Tóm tắt những kết luận mới của luận án

1. Nghiên cứu được tiến hành trong 5 năm (2013 – 2017), là công trình đầu tiên tại Việt Nam phân tích các đặc điểm dịch tễ và lâm sàng trên 101 bệnh nhân người lớn mắc tiêu chảy do Clostridium difficile điều trị nội trú tại bệnh viện Bạch Mai. Bệnh gặp ở nam nhiều hơn nữ (tỉ lệ nam: nữ là 1,7:1), ở hầu hết các tỉnh/ thành Miền Bắc Việt Nam (21/28 tỉnh/ thành phố), nhiều hơn ở người trên 60 tuổi (49,5%). Triệu chứng lâm sàng thường gặp là sốt, đau bụng, chướng bụng, tiêu chảy từ 3-6 lần/ ngày, tăng bạch cầu và procalcitonin máu. Tử vong chung tại viện là 5,9%, ở bệnh nhân hồi sức tích cực là 14,3%.

2. Các yếu tố nguy cơ mắc tiêu chảy do C.difficile ở người lớn trong phân tích đa biến bao gồm tuổi cao ≥ 65 tuổi (OR=2,01), sống ở thành thị (OR=1,76) và bệnh nhân lọc máu chu kỳ (OR=7,32). Yếu tố liên quan đến chẩn đoán tiêu chảy do C.difficile là đại tiện phân có nhầy mũi và tiêu chảy 7-10 lần/ ngày. Dùng kháng sinh nhóm glycopeptid điều trị bệnh khác trong vòng 8 tuần trước khi tiêu chảy là yếu tố bảo vệ với bệnh.

3. Nghiên cứu tìm thấy cả chủng C.difficile mang gen độc tố A+B+ và A-B+. Bệnh nhân tiêu chảy do C.difficile mang gen độc tố A+B+ chiếm 49,5%, A-B+ là 44,6% và mang cùng lúc hai chủng có độc tố A+B+ và A-B+ là 5,9%. Phát hiện sự lưu hành 8 kiểu gen ribotype của C.difficile gồm: 2 kiểu gen trf 017 mang gen độc tố A-B+; 6 kiểu gen cc835, og39, 014, ozk, cr001 mang gen độc tố A+B+.

SUMMARY OF NEW FINDINGS OF THE THESIS

1. This is the first study in Vietnam on epidemiological and clinical characteristics of Clostridium difficile diarrhea, which was conducted in 5 years (2013 – 2017), on 101 adult patients with confirmed Clostridium difficile diarrhea at Bach Mai Hospital.The disease was seen more in men than in women (with male to female ratio of 1.7: 1); the patients came from almost all the provinces of the North of Vietnam, with the predominance of people aged over 60 years (49.5%). Most common Clinical symptoms of C.difficile diarrhea include fever, abdominal pain, abdominal distention, diarrhea 3-6 times/day, elevated white blood cell count and procalcitonin. The mortality was 5.9% among patients with C.difficile diarrhea, higher if the patients were from intensive care unit (14.3%).

2. A multi-variable logistic regression analysis showed that the risk factors for Clostridium difficile diarrhoea in adults included: age over 65 years (OR=2.01), living in the cities (OR=1.76), and having chronic renal failure requiring dialysis (OR=7.32). The related factors for the diagnosis of Clostridium difficile diarrhea were the presence of mucus in feces and frequency of diarrhea of 7-10 times per day. The factor protecting from Clostridium difficile diarrhea was using glycopeptide to treat another disease within 8 weeks before diarrhea.

3. The study found both A+B+ and A-B+ toxigenic C.difficile strains, with 49.5% of the patients harboring the A+B+ strains, 44.6% harboring the A-B+, and especially 5.9% carried both A+B+ and A-B+ strains. Eight PCR ribotypes of C.difficile was identified: two ribotypes trf and 017 in toxigenic C.difficile A-B+ strains, and six ribotypes cc835, og39, 014, ozk, cr and 001 found in toxigenic C.difficile A+B+ strains.
