Tóm tắt những kết luận mới của luận án:
- Luận án cho thấy năng lực tự chăm sóc của người bệnh đái tháo đường tại các trạm y tế phường, thành phố Hồ Chí Minh ở mức thấp, với 24,9% người bệnh có năng lực tự chăm sóc tốt. Trong 5 lĩnh vực tự chăm sóc, tuân thủ dùng thuốc đạt mức tốt cao nhất (89,7%); các lĩnh vực còn lại đều dưới 50%, trong đó tự theo dõi đường huyết thấp nhất (3,0%). Các yếu tố hỗ trợ cho hoạt động tự chăm sóc cũng còn hạn chế: 37,5% người bệnh có kiến thức tốt, 18,5% có mức tự tin cao, 19,5% nhận được nhiều hỗ trợ từ nhân viên y tế và 7,5% nhận được nhiều hỗ trợ từ gia đình/bạn bè.
- Luận án xác định một số yếu tố liên quan đến năng lực tự chăm sóc tốt của người bệnh đái tháo đường, bao gồm: thời gian điều trị tại trạm y tế lâu hơn (aOR = 3,38; KTC 95%: 1,41–8,09), kiến thức cao hơn (aOR = 1,10; KTC 95%: 1,01–1,20), mức tự tin cao hơn (aOR = 2,50; KTC 95%: 1,93–3,23), nhận được nhiều hỗ trợ hơn từ nhân viên y tế (aOR = 2,42; KTC 95%: 1,47–3,97) và gia đình/bạn bè (aOR = 1,55; KTC 95%: 1,12–2,13), và kiểm soát đường huyết tốt (aOR = 2,33; KTC 95%: 1,29–4,20).
- Luận án đã xây dựng và triển khai chương trình can thiệp đa thành phần gồm giáo dục sức khỏe theo nhóm cho người bệnh và gia đình, hỗ trợ bằng tin nhắn qua ứng dụng di động Zalo và cung cấp tài liệu hướng dẫn tự chăm sóc, nhằm nâng cao năng lực tự chăm sóc của người bệnh đái tháo đường tại trạm y tế. Chương trình can thiệp đã giúp cải thiện rõ rệt các hành vi tự chăm sóc, gồm chế độ ăn (+36,5%; +1,36 điểm), hoạt động thể lực (+20,7%; +1,29 điểm), tự theo dõi đường huyết (+0,43 điểm), chăm sóc bàn chân (+34,9%; +2,84 điểm), tuân thủ dùng thuốc (+8,9%) và năng lực tự chăm sóc chung (+39,0%; +1,35 điểm). Đồng thời, chương trình cải thiện kiến thức tốt (+49,2%; +6,6 điểm), mức tự tin cao (+28,0%; +1,88 điểm), hỗ trợ nhiều từ nhân viên y tế (+29,0%; +0,8 điểm) và từ gia đình/bạn bè (+21,4%; +0,75 điểm), đồng thời giúp giảm chỉ số huyết áp tâm trương 5,17 mmHg và giảm chỉ số đường huyết lúc đói 12,38 mg/dL.

SUMMARY OF NEW FINDINGS OF THE THESIS
- The thesis demonstrates that the self-care capacity of patients with diabetes mellitus receiving care at ward health stations in Ho Chi Minh City was generally low, with only 24.9% of patients demonstrating good self-care capacity. Among the five domains of self-care, medication adherence had the highest proportion of patients achieving a good level (89.7%), whereas all other domains were below 50%; self-monitoring of blood glucose was the lowest (3.0%). Factors supporting self-care were also limited: 37.5% of patients had good knowledge, 18.5% had a high level of confidence, 19.5% received a high level of support from healthcare professionals, and 7.5% received a high level of support from family members or friends.
- The thesis identifies several factors associated with good self-care capacity among patients with diabetes mellitus, including a longer duration of treatment at the health station (aOR = 3.38; 95% CI: 1.41–8.09), higher levels of knowledge (aOR = 1.10; 95% CI: 1.01–1.20), higher levels of confidence (aOR = 2.50; 95% CI: 1.93–3.23), greater support from healthcare professionals (aOR = 2.42; 95% CI: 1.47–3.97) and from family members or friends (aOR = 1.55; 95% CI: 1.12–2.13), and good glycemic control (aOR = 2.33; 95% CI: 1.29–4.20).
- The thesis developed and implemented a multicomponent intervention program comprising group-based health education for patients and their family members, message-based support through the Zalo mobile application, and the provision of self-care guidance materials to improve self-care capacity among patients with diabetes mellitus receiving care at health stations. The intervention resulted in marked improvements in self-care behaviors, including diet (+36.5%; +1.36 points), physical activity (+20.7%; +1.29 points), self-monitoring of blood glucose (+0.43 points), foot care (+34.9%; +2.84 points), medication adherence (+8.9%), and overall self-care capacity (+39.0%; +1.35 points). In addition, the intervention improved the proportion of patients with good knowledge (+49.2%; +6.6 points), a high level of confidence (+28.0%; +1.88 points), a high level of support from healthcare professionals (+29.0%; +0.8 points), and a high level of support from family members or friends (+21.4%; +0.75 points). The intervention also contributed to reductions in diastolic blood pressure by 5.17 mmHg and fasting blood glucose by 12.38 mg/dL.
